For your convenience, you may complete the online request for an appointment, or you may call the office. When calling to make your first appointment please have the following information on hand:

• All Insurance cards, primary and secondary

• Your social security number and/or the primary policy holder's numbers (if applicable)

• The name and number of the referring doctor

Please call the office to make an appointment in advance. In the event that you can not reach an appointment specialist, please leave a message and we will try our best to contact you within one business hour.

If you cannot make your appointment please call and reschedule within twenty-four hours of scheduled appointment to allow other patients to be served.
